How the assessment process works
The Clear Focus assessment is completed over two structured, face-to-face appointments in Morayfield:
Session 1 — Assessment
30 minutes | Face - to- face
Detailed clinical assessment of symptoms, medical history, and current circumstances. Standardised questionnaires and rating scales are completed. A physical examination may be conducted where clinically indicated.
Session 2 — Diagnosis & Plan
30 minutes | Face-to-face
Review of assessment findings, diagnostic discussion, and development of a personalised care plan. A summary letter is sent to your regular GP with your consent.
Sessions are typically scheduled a few weeks apart. No referral is required to book — self-referrals are welcome.
