The Adult ADHD Assessment Process

A clear, structured two-session approach to understanding your symptoms and creating a personalised care plan.

The Clear Focus ADHD assessment is a structured, two-session process conducted in person at Health Hub Morayfield. The process follows current Australian ADHD clinical practice guidelines and uses standardised clinical tools and questionnaires.

A referral or medical summary from your regular GP, if available, is useful to bring to your first appointment — but it is not mandatory.

What to do before you attend

To make the most of your appointment time, there are a few things you may wish to prepare:

If you have a GP, ask them for a medical summary letter. This is not mandatory, but it may be helpful.

Gather any previous reports, referral letters, or diagnosis documents from psychologists, psychiatrists, or paediatricians — even if dated

Note the medications you are currently taking, including dosage and prescribing practitioner

You will be sent online questionnaires to complete at home before your second appointment

Session 1:
Clinical Assessment

Duration: 40 minutes | Format: Face-to-face, Health Hub Morayfield

Your first appointment is a team-based assessment. A nurse gathers your history and measures your vitals, performing an ECG where indicated. Dr Jeremy then conducts the clinical assessment using a structured approach informed by standardised tools and diagnostic criteria.

Please bring any previous reports or letters relevant to your mental health or ADHD history. Even older documents can provide useful context for your assessment.

Session 2:
Diagnostic Discussion & Care Plan

Duration: 40 minutes | Format: Face-to-face, Health Hub Morayfield

Your second appointment brings together the information gathered in Session 1, including any completed questionnaires and investigation results, to inform a diagnostic discussion and care planning conversation.

What happens during Session 2:

Regarding medication:

Not everyone who is assessed for ADHD will be recommended medication. If medication is discussed as part of your care plan, this will be based on a clinical assessment of the potential benefits and risks in your individual circumstances, your preferences, and Queensland prescribing requirements. Medication initiation and management, where appropriate, may be provided by Dr Jeremy in line with current Queensland prescribing requirements.

Ongoing care after your assessment

After the initial two-session assessment process, ongoing care options will be discussed based on your individual circumstances. These may include:

If you already have an ADHD diagnosis from a psychiatrist or psychologist and are seeking ongoing GP-led management, the clinic may be able to support continuity of your care, depending on your circumstances.

Dr Jeremy will request a copy of your specialist’s diagnosis letter and relevant clinical details before commencing management.

Assessment Standards

The Clear Focus assessment process is aligned with the Australian Evidence-Based Clinical Practice Guideline for ADHD published by the Australian ADHD Professionals Association (AADPA).
